Our Services
We offer four core programs across northern New Jersey, all covered by NJ Medicaid or FamilyCare for eligible participants.
Our home care program provides personal care aides (PCAs) who assist with bathing, grooming, dressing, meal preparation, and medication reminders for NJ Medicaid-eligible individuals. Services are authorized through the NJ Division of Medical Assistance and Health Services (DMAHS) and managed by our licensed registered nurses. We serve clients across Essex, Hudson, Bergen, Passaic, and Union counties with flexible scheduling including evenings and weekends.
NJ Medicaid CoveredThe NJ Division of Developmental Disabilities (DDD) funds community-based services for individuals with intellectual and developmental disabilities. Priority Groups NJ is an approved DDD provider offering community living supports, day habilitation coordination, and individualized supports. Our team works directly with Support Coordinators and families to build person-centered plans that align with each individual's goals and NJ DDD budget allocations.
DDD WaiverFamily caregivers need rest. Our respite care program provides short-term relief for unpaid family caregivers by placing trained aides in the home for planned or emergency periods. Services are available through the NJ Division of Developmental Disabilities respite program and through self-directed Medicaid budgets. We offer both in-home respite and facility-based overnight respite placement coordination for families in Newark, Jersey City, Hackensack, and surrounding areas.
DDD / MedicaidEmployment is central to community inclusion. Priority Groups NJ provides job coaching, job development, and workplace support services for DDD-enrolled individuals seeking competitive integrated employment. Our supported employment specialists partner with local businesses across Essex and Hudson counties to identify job opportunities that match each person's strengths and career interests. Services are funded through the NJ DDD Employment Supports program and Division of Vocational Rehabilitation (DVR).

Medicaid Accepted
All of our core services are available to NJ Medicaid-eligible residents at no out-of-pocket cost. Here's what Medicaid covers with Priority Groups NJ:
Our home care PCA program is covered for income-eligible adults who meet functional requirements determined by a nurse assessment. We accept all major NJ Medicaid managed care plans including Aetna Better Health, Horizon NJ Health, Amerigroup, and WellCare.
Individuals enrolled with the NJ Division of Developmental Disabilities receive services through a Medicaid waiver budget. We work directly with your Support Coordinator to ensure services align with your approved ISP and annual budget allocations.
For seniors and adults with physical disabilities, we work with Managed Long Term Services and Supports (MLTSS) plans and the Jersey Assistance for Community Caregiving (JACC) program to coordinate home care services for qualifying individuals.
If you or your family member participates in NJ's self-directed budget program, Priority Groups NJ can serve as your fiscal/employer agent or as a vendor within your approved plan — giving you more control over who provides your care.
Getting Started
Applying for Medicaid home care in New Jersey is a multi-step process. We guide you through every phase — at no cost to you.
Reach our NJ intake team at (201) 305-0936 to begin your eligibility review. We'll ask about your Medicaid status, county of residence, current diagnosis or disability, and care needs. If you're not yet enrolled in NJ Medicaid, we can connect you with a benefits navigator who can assist with the application process — there is no charge for this service.
Once we confirm your eligibility, a registered nurse will conduct an in-home assessment to document your functional needs. Our care coordinators then submit your service authorization request to the appropriate NJ agency — DMAHS, your managed care organization, or the DDD Support Coordination unit. We handle all documentation, follow-up calls, and appeals if needed.
After authorization, we match you with a trained, background-checked aide who fits your schedule, language preference, and care requirements. Your assigned RN supervisor conducts regular check-ins to ensure care quality and adjust hours as needed. Most clients in Essex and Hudson counties begin services within 5–10 business days of receiving authorization.
