Priority Cares provides specialized Medicaid waiver services for New Yorkers living with traumatic brain injury, physical disabilities, and complex medical needs — so you can live independently, in your own home.
New York State offers two Medicaid waivers for individuals who need long-term home care support. Priority Cares is approved for both.
Nursing Home Transition & Diversion — For individuals with physical disabilities who want to live in the community instead of a nursing facility.
Traumatic Brain Injury — For individuals 18–64 who have survived a TBI and need support to remain in the community.
How the process works: Referrals for NHTD and TBI waiver services are coordinated through the Regional Resource Development Centers (RRDCs). Our care coordinators work directly with RRDCs to streamline your enrollment and get services started quickly. If you've already been evaluated, we can often begin within days of approval.
You may be eligible for NHTD or TBI waiver services if you meet the following criteria:
You must be enrolled in NY Medicaid. We can help you apply if you're not yet enrolled.
Acquired brain injury from trauma, stroke, or other causes. Must need home-based support to remain in the community.
18+ with physical disability risk of nursing home placement who want to remain in or return to community living.
You want to live in your own home, apartment, or a community setting — not a nursing facility.
An RRDC assessment will determine the level of services you need. We accompany you through this process.

We refer you to the appropriate Regional Resource Development Center. Our team prepares your documents and attends the assessment with you (or advocates on your behalf).
Once approved, your Service Coordinator and Priority Cares develop a personalized care plan — matching you with the right caregiver and services for your needs.
Your caregiver starts. You receive regular check-ins from your coordinator. Services adjust as your needs change.
Our NHTD/TBI team will call you within 2 hours (Mon–Fri, 9 AM–6 PM). No paperwork needed for the first call.
